Fine Lines & Skin Laxity

Lines from movement and lines from volume loss need opposite treatments.

Two different things get called wrinkles. Lines that appear when you frown or smile are made by muscle, and relax with a neuromodulator. Lines and folds that are there at rest come from lost volume and collagen, and need a stimulator or resurfacing. Treating one as if it were the other is the most common reason people are disappointed by a result.

01

Why it happens

  • Repeated muscle movement folding the same spot until the fold persists

  • Collagen production declining from the mid-twenties, roughly one percent a year

  • Facial fat pads shrinking and descending, which reads as sagging rather than as volume loss

  • UV exposure, which degrades existing collagen — the single largest controllable factor

Common questions

Do I need Botox or filler?

Look in a mirror with a relaxed face. If the line disappears, it is made by movement and a neuromodulator such as Botox or Xeomin is the right treatment. If it is still there, it is volume loss, and a collagen stimulator such as Sculptra or a filler is what addresses it. Many people need both, in different areas.

What age should I start?

When a line starts to remain visible after your face relaxes — that is when movement has begun to leave a permanent crease, and treating it then is easier than reversing it later. That is late twenties for some people and forties for others, and chasing a number rather than what your own skin is doing is not useful.

What helps skin that looks thin and crepey?

That is usually barrier and collagen quality rather than volume, so a polynucleotide skin booster such as Rejuran is often the better fit than a filler — it works on elasticity and skin repair rather than adding shape. Fractional laser also helps, with more downtime.

Can anything replace a facelift?

No. Injectables and lasers work on skin quality and volume; they do not lift descended tissue. They can delay the point at which surgery becomes the only option, and they suit mild to moderate laxity well. For significant sagging, a surgeon is the honest referral.
